Patients are seen by appointment – on the same day the appointment is made if possible. To schedule an appointment, please call (303) 788-3100 during regular business hours.
Please call in advance for office visits. Make follow-up appointments as you leave. We make every effort to stay on schedule although emergencies do arise. If we are seriously delayed, we attempt to notify patients beforehand.
New patients are asked to arrive 15 minutes early to complete paperwork. You can download, print, and complete the forms prior to your appointment. Please bring the completed forms, a list of all medications you are currently taking, a photo ID, and your insurance card to your appointment.
We require payment of your co-payment and past-due account balances at the time of service. We accept cash, checks, debit and all major credit cards.
We will submit claims to most major insurance carriers. Please bring your insurance card and I.D. with you to every visit so we can ensure that our records are accurate.
We recommend that you call your insurance company to verify that we participate with your plan.
Please notify us if you have any change in your name, phone number, address, or insurance as soon as it occurs. And please be prepared to present your insurance card at every visit.
Cancellation, Late and Dismissal Policies
Cancellation and Late Policy
We understand that unplanned issues come up and that sometimes you will need to cancel appointments.
However, we ask that if you are unable to keep an appointment, please call our office at least 24 hours ahead of time so that the appointment time can be made available to other patients who require care.
Please be aware that patients who are late or miss a scheduled appointment may be dismissed from our practice.
At Swedish Family Medicine, we are committed to providing each patient with the highest quality care while extending the utmost respect for each individual and their needs. In return, we also ask that our patients and their loved ones extend this same courtesy to our staff and physicians. Patients who demonstrate non-compliant, rude or disruptive behavior may be dismissed from our practice.
For all prescription refills, patients should contact their pharmacies directly. Your pharmacies will in turn contact our office for refill information, if necessary. Patients should allow three business days to process all prescription refill requests.
After-hours & emergencies
If you are experiencing a medical emergency, dial 911. For urgent needs that occur outside of regular office hours, patients may call (303) 788-3100. Calls will be forwarded to Swedish Family Medicine's answering service, which will contact the doctor on call.
Please do NOT call this number for prescription refill needs or to request antibiotics for a condition that requires a doctor's visit. Controlled substances cannot be prescribed after hours.
